NO SHOW POLICY:
We reserve the right to charge $25 NO SHOW FEES for ESTABLISHED patients and $50 for NEW patients.
CANCELLATION POLICY:
Please call at least 24 hrs in advance to cancel or reschedule an appointment to avoid a $25 late cancellation fee.
REFERRAL POLICY:
If your insurance requires a referral to see the specialist, please obtain a referral from your primary care doctor and bring it to your appointment to void receiving a bill for the services provided by the doctors at NJSS.
NOTE: In most cases, a referral is NOT the script provided by your doctor to see the specialist. A REFERRAL is what is required by your insurance to see the specialist, this will list the number of visits, with effective and expiration dates. If you are unsure you may ask your primary doctor if your insurance requires a referral or not to avoid any unnecessary charges or bills from NJSS.
PROCEDURE AND APPOINTMENT REPORTING TIME:
If you are a new patient or you are coming in for a procedure please report at least 15 minutes prior to your scheduled appointment to avoid unnecessary delays or possible cancellation of your appointment.
LATENESS:
We understand emergencies happen. However, if you are late more than 30 minutes your appointment may be canceled and rescheduled.
